Help with strange clutch problem

Hi all, I have a civic ek4 vti and recently I have been having a strange problem with my clutch. It was fine and then one day after about 2 hours of motorway driving and getting off at the junction,when I went to pull away and the whole car started shaking quit bad.I carried on driving and when trying to shift into any gear after about 3000 rpm it will be quite difficult to get into gear, sometimes even crunching. The gearbox was smooth as anything before this so Im quite sure thats not the problem. Now ive dealt with the problem for the past week or so and its progressively getting worst. Ive not had any slipping, just shaking of the car when pulling away, which hasn't been as bad recently, but the gear crunching is getting alot worst. It kind of feels like the clutch isn't fully releasing itself from the flywheel, I also think the actual feel of the clutch pedal has changed. When driving it now its difficult to change gears, to get it into gear I have to hold the stick in position for a second then it will pop in. I have checked my master cylinder and slave cylinder for leaks and found none. I have also bled to clutch to see if there was any trapped air but this also has not helped. Ive been told it could be the release/throwout bearing but just looking for any other opinions about it. any help appreciated, cheers
 
Ive had this twice before, once was a pressure plate tab snapping allowing the thick steel ring to move off centre,second time was friction plate spring moving around,

Time for a new clutch!
